On Nov. 17, 2025. an individual in California used my details to book a telehealth appointment with a physician assistant at the Carbon Health clinic in San Francisco. The patient identified himself using a common misspelling of my name), and provided my date of birth, home address, and insurance details. I do not know where he got any of this information. The impostor kept his appointment on Nov. 17, impersonating me, and got a prescription for hydroxyzine. When I contacted the medical provider, I was able to learn that the impostor gave the phone number 747-242-8101. This phone number is a burner phone or is no longer in service, but I found it on the website hopespringswellnessinc.com, which also appears to be a scam. The hopespringswellnessinc.com site is not affiliated with the real Hope Springs Wellness Center (*****************) but it uses their logo. The scam site appears to be collecting insurance details as part of this scam too: they have a Verify Insurance page which invites people to send their health insurance information. Please note that this is not a complaint against a real business. This is regarding an impostor who committed identity theft and who apparently runs a scam website mimicking a real company's site.
