Please do not provide this entity with my mailing address that is part of this complaint; I’m already a victim of identity theft and my victim’s statement is a part of my credit profile. On 12-30-25, I received a phone call/voicemail regarding an alleged case in which I’m allegedly a part. They used my married name, ******, but I’ve been divorced for over twenty years and legally resumed my maiden name at that time. Below is the information from the voicemail “Proof Service Center” left on my phone: “This message is intended for ***** ****** with the reference number 25 dash 00. This message is intended for ***** ****** with the reference number 25-013219. My name is Crystal Smith from the Proof Service Center. I'm calling to verify you'll be available to accept and sign for the legal documents in regards to a legal matter you've been named in. Please contact her o?ce with your statements at 844-220-5137. You have been notified” I have no idea what this is about, but would like your o?ce to investigate this issue as I am a victim of identity theft and have had both a “Fraud Alert@ and have had my credit profile frozen for over ten years due the identity theft issues. I would like to find out what this is about or if it’s a scammer for information. I hung up their call when they started asking for personal information such as my social security number, etc., and chose to file a complaint with your with the Attorney General’s office in my state of, Arizona, to get to the bottom of this matter and it was recommended that I file a complaint with your office as well. This business did not display or give an address for their business. Also, for my safety please do not provide this entity with my mailing address as I’m a victim of identity theft.
