C.P.S. sent me a fraudulent form trying to scam me for money. They sent what looked to be a government document with all my business LLC information on it, stating I needed to complete the form and send a check for $160 by 3/20/2026. The document even had an Indiana Code 23-0.5-2-13. It states the form is for the State of Indiana Business Entity Report Filing (2026- Business Entity Report Instruction Form). There's a 317-981-1664 and a separate piece of paper that has a 877-329-4662 phone number. They want the form and check mailed to C.P.S.- 5868 E. 71st Street, Suite E #109, Indianapolis, IN 46220, which shows it's a Mailing Station when I looked it up. They know everything about my LLC, including my home address. I fill out everything I can on the actual IN Gov website, and the rest I leave up to my CPA. I wonder how many people they have scammed.
