It had my full name and address and business name. The mailing address to send the check is C.F.S. A letter in the mail to me, an owner of an LLC, that is titled "2026 – Annual Renewal Instruction Form". Says to complete the form and sent $135 via the provided return envelope. The envelope is addresses to: C.F.A., PO Box 4256, St Paul MN 55101. There is another small flyer with the form that is titled "Filing Checklist" and has a phone number of it: 877-329-4662. The form is partly completed with my business and personal information (address, business name). Front of the form is english, the back is in spanish.
