Bakarika Corp filed a fraudulent claim TWICE with my Medicare advantage insurance plan. Amount being billed on both claims is the same, $12,062, for medical supplies. I have never heard of this company and have never ordered or received the medical supplies and services billed to my insurance company. I have none of the medical conditions and have never been treated for any of the medical conditions associated with these supplies and services. I reported the first claim dated December 4, 2025, to my insurance company and the Medicare Fraud Hotline and now a second identical claim, also dated December 4, 2025, has been submitted to my insurance company and was received April 15, 2026. I have lost or shared none of my private information (Medicare, social security, insurance cards, driver's license, etc.) and never put this information online. I am attaching details from the first claim, which was ultimately denied because my insurance company had not received requested information from the provider (Bakarika Corp). I researched this corporation and they appear to be a resurrection of another company with a similar name and same owner (Bakari Chakhmakhashvili) and was indicted for Medicare fraud.
