They said they canceled United Healthcare card and sending a Humana card that begins March 1st. They asked for my personal name, address, birthday, email and health insurance ID number. They asked about which providers I see and the facilities they work at. In addition, they asked what medications I take. They also had me call back this number, 833-556-0879.
