I called about home health care for my dad and Info from veterans association gave me: 1. I will go to vetrecs.archives.gov to get dads DD214 form which indicates his length of service etc. 2. I will sign him up for health benefits from veterans if he does not have this already. 8772228387 or va.gov 3. I will have his primary doctor fill out his aide and attendant form 4. The form goes directly to a geriatric social worker who will determine what he needs.. The price for care is determined by hours needed and last years income. But it will be less than the $180 a day we were just quoted from a friend not an agency. We are in MA u.s.a and she said the agency she works for would be $25 an hour if we went through them.
