Ourpetpolicy.com is trying to get people with service animals that are renewing or signing a new lease agreement with their property management to sign a ROI(release of information) so that they can contact our doctor and ask our doctor several illegal questions about our service animals and the service animal letter and etc. It is illegal to make it mandatory to sign an ROI after you've already provided a legitimate service animal letter from a health professional to property management. And the questions on the questionnaire that they make the health provider fill out and sign are also illegal according to HUD. I have attached a screenshot of the questionnaire.
